Fill out this page according to the instructions on the Information page. This page is designed and sized so that the fields above will fit on one page of A4 paper if printed out from a browser. Either fill it in on the browser and then print out or print it out and then fill it in by hand. The Clear Form button clears the whole page.

* If you have Infants and Juniors schools that are part of the same primary school but use slightly different character sets, fill out a form for each of them and send them in as the same order. Instead of charging £15.00 each for them, I will only charge £22.50 for the total order. Make sure that each font has a unique name - XX Infants and XX Juniors for example.
